OSHA Confined Space Entry/Rescue Hazmat
Confined spaces incidents aren't the most common, but they can be the most deadly. That's because the hazards tend to be misunderstood or underestimated. And many confined spaces fatalities involve not one, but two, victims: the worker and the rescuer.
When the following conditions, or similar conditions, may develop as a consequence of a release of hazardous substances or threat of release, such situations would be considered emergency situations requiring an emergency response effort:
  • High concentrations of toxic substances.
  • Immediately Dangerous to Life and Health (IDLH) environments.
  • Situations that present an oxygen deficient atmosphere.
  • Conditions that pose a fire or explosion hazard.
  • Situations that require an evacuation of the area.
  • Situations that require immediate attention because of the danger posed to employees in the area.
Fall Protection Systems Lock Out / Tag Out
The US Department of Labor (DOL) lists falls as one of the leading causes of traumatic occupational death, accounting for eight percent of all occupational fatalities from trauma. Any time a worker is at a height of four feet or more, the worker is at risk and needs to be protected. Fall protection must be provided at four feet in general industry, five feet in maritime and six feet in construction. However, regardless of the fall distance, fall protection must be provided when working over dangerous equipment and machinery.
"Lockout/Tagout (LOTO)" refers to specific practices and procedures to safeguard employees from the unexpected energization or startup of machinery and equipment, or the release of hazardous energy during service or maintenance activities. This requires that a designated individual turns off and disconnects the machinery or equipment from its energy source(s) before performing service or maintenance and that the authorized employee(s) either lock or tag the energy-isolating device(s) to prevent the release of hazardous energy and take steps to verify that the energy has been isolated effectively.
